SACS is excited to hold the flu shots again at our district onsite health clinic!
  To better manage the lines, we are scheduling times for the 2 day “flu shot” event.  Please review the following schedule and sign up for your flu shot with your building secretary.  Remember, for plan participants, they are free!  If you are unable to attend at your requested time, you may attend at another time.

The following criteria MUST BE MET to receive the Flu Vaccine:

1.  Sign name on Sign up sheet located in each school's office.

2.  Sign Release Form (they will be available at the clinic when you receive the vaccine).

3.  No cost to employees and dependents on SACS medical plan.  (must show insurance card to confirm coverage)    

4.  Employees and dependents without SACS medical insurance pay the fee of $27.00.  (correct change or check payable to SACS)

 

THE VACCINE - This is a sterile product for intramuscular injection composed of antigens of the strains of influenza virus recommended for vaccine use during the 2018-19 season by the U.S. Center for Disease Control.

 

RISKS AND POSSIBLE SIDE REACTIONS -
Side effects of influenza vaccine are generally mild in adults and occur at low frequency.  These reactions consist of tenderness at the injection site, fever, myalgia, malaise and muscle aches.  These symptoms occur within 6-12 hours after immunization and last up to 48 hours.

 

SPECIAL PRECAUTIONS – Please refer to the attached Influenza Vaccine Statement.

PERSONS WHO HAVE A SEVERE ALLERGIC REACTION TO EGGS SHOULD NOT RECEIVE THIS VACCINE.

PERSONS WITH FEVER OR ACTIVE INFECTION OF ANY KIND SHOULD NOT RECEIVE THIS VACCINE.  Persons who have received another type of vaccine within the past fourteen days should contact their personal physician before receiving this vaccine.
